Endoscopic Ilipsoas Release

What is the iliopsoas?

The iliopsoas muscle is a key player in hip function. It originates at the lower spine and attaches to the upper end of thighbone (femur).  It is responsible for flexing and rotating the hip. When this muscle becomes tight or inflamed, called iliopsoas tendonitis, it can cause pain in the groin, hip and low back.

An endoscopic iliopsoas release is a minimally invasive surgical procedure that releases the tension on the muscle caused by impingement, tears, and other conditions. The result is pain relief and, in some cases, restoration of normal function. Utilizing the latest technology, clinicians at LALL Orthopedics+ have helped pioneer minimally invasive procedures to treat iliopsoas tendonitis whilst preserving the hip joint.

During an endoscopic iliopsoas release, a small 4-millimeter camera is used to visualize the hip joint. The surgeon then uses special instruments to release the thickened and scarred tissue around the iliopsoas tendon to improve range of motion and relieve pain. This technique allows for a more precise and less invasive surgery, with shorter post-operative stays and faster recovery times. It is typically performed as outpatient surgery meaning that patients can go home the same day.

Endoscopic iliopsoas release can be an effective treatment for many different conditions that cause hip pain, including:

  • Internal snapping hip – a condition characterized by an audible pop or snap when the iliopsoas tendon moves over its groove in the hip socket. It is common and can be very painful with hip movements. The gold standard treatment is conservative management, but when this doesn’t relieve pain or other damage is present, endoscopic iliopsoas release is warranted.
  • Iliopsoas impingement – Iliopsoas impingement involves damage to and compression of the labrum that lines the hip socket caused by a tight or inflamed iliopsoas tendon. Impingement may be the cause of hip pain after total hip replacement. Surgery removes the impingement and improves joint biomechanics.
  • Articular cartilage damage
  • Synovitis (inflammation of the hip joint)
  • Iliopsoas Tendonitis (inflammation of the tendon)
